BAND PHYSICAL FORM

The Vestavia Hills Board of Education mandates that all band and auxiliary students must submit a band physical completed by a licensed medical practitioner.

WELLNESS TEAM AUTHORIZATION FORM

The Rebel Band Wellness Team is only authorized to provide first aid. In emergency situations, the Wellness Team will contact EMS (911) immediately. In order to provide the best support, families are encouraged to complete the medical form.

MEDICATION FORM

If your student must take medication during band camp or any band related event outside of school hours (away games, competitions, performances), then you must:

Have a licensed medical professional complete the Alabama Dep of Education Medication Authorization
Turn in the medication and from to the band directors.

If your student experiences a significant medical event, the band will require a doctor's clearance before resuming participation. You must:

Have a licensed medical professional complete the Return to Practice Authorization
Turn in the authorization form to the band directors.
